Required Surveys
Required by Grantors - Survey data from events, demonstrations, awareness campaigns, presentations, and ALL uses by individual(s) and organizations. AAW's services are provided only by fulfilling grantors' requirements. This is required data.

Individuals-Family-Friends (Social Connections) - Choose this survey if an individual in your group or you (our client) used an outdoor wheelchair (OWC). Please help us keep this program for your community by submitting your completed survey.
Community Survey* - Choose this survey to help measure the impact of awareness campaigns, presentations, and equipment used at an event, activities, or demonstrations. This is completed by leaders, organizers, and AAW for community activities, events, and ...

Surveys are to be completed after using the equipment either prior to or when the equipment is returned. This requirement must be completed to have the deposit returned per AAW Policy.
